Comments:
An unannounced complaint investigation was initiated and concluded on 01/11/2024 in response to a complaint received by the licensing office on 01/10/2024 regarding supervision. Interviews were conducted relating to the allegations on 01/11/2024. The information
gathered during the investigation does support the allegation, so the complaint is determined to be "valid". If you have any questions, contact the licensing inspector at (757)354-7307.

Description: COMPLAINT RELATED
Based on observation and interview it was determined the center did not ensure that children under the age of 10 years of age were always within actual sight and sound supervision of staff.
Evidence:
1. Inspector observed a child alone in the open area outside of the restroom during this inspection. Director noted that staff #1 was responsible for supervising that child.
2. Several staff members reported that they were aware that child #1 has a history of "showing himself to other children and having them show themselves to him".
3. Several staff members stated that child #1 and child #2 are now in sperate classes and cannot play with each other due to these issues.
4. Staff #4 was observed working on the computer in the classroom while the 11 children in the class were playing. Staff #4 was the only staff in the room and could not observe all of the children in the classroom from seated position behind the computer.

Plan of Correction: 1. Staff #1 was observed supervising the child from an obstructed view. We will use the suggested placement as suggested by the inspector.
2. In an effort to properly supervise the child, supervision measures were in place
for the child that was showing them-self toothers. All staff were aware and apart of
the plan. The Plan of Action was implemented before the inspection and will
continue to be enforced. Corrected before inspection.
3. We felt separating them was important for the safety of both children. All staff have been informed and advised of the plan. The Plan of Action was implemented before the inspection and will continue to be enforced. Corrected before inspection.
4. Staff #4 has been advised and coached on the sight and sound requirement.

Complaint related: No
Description: Based on observation and interview it was determined the center did not ensure that when in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group shall apply to the entire group.
Evidence:
1. There were 11 children, with the youngest being under the age of school eligibility in the room with staff #4 during this inspection requiring a ratio of 1 staff person for every 10 children aged 3 to school age.
2. Director confirmed that this class was out of ratio during this inspection.

Plan of Correction: Feeling the need to do what was best and safest for the children, the child was
relocated to the KG classroom. We overlooked that the child did not turn 5 until after September 30 which changesthe ratio of the classroom. We have added
a second staff to this class at all times the child is there.
