Comments:
A monitoring inspection was conducted on Friday, February 2, 2024. The inspection began at approximately 9:20 AM until 11:20 AM. There were 14 children present, ranging in ages from 13 months to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-160-C
Description: Based on review of records and interviews, the center failed to ensure that staff had a tuberculosis (TB) screening at learning every 2 years from the initial screening.

Evidence:
1. The record for staff #2 contained a tuberculosis (TB) test dated for 12/15/2021. An updated tuberculosis (TB) test or screening should have been updated on or before 12/15/2023.
2. The program director stated that staff #2 did not have a current tuberculosis (TB) test.

Description: Based on review of records and interviews, the center failed to ensure that staff had a tuberculosis (TB) screening at learning every 2 years from the initial screening.

Evidence:

Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence: The soft large blocks in the main play area, were torn exposing the inside materials presenting a choking hazard.

Description: (Repeat Violation) Based on observation, the center did not ensure that hazardous substances such as cleaning materials were kept in a locked place.

Evidence:
1. There was package of Clorox wipes stored in the area next to the children?s lunch boxes in the main play area.
2. A bottle of hydrogen peroxide was stored on top of the microwave in the kitchen.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation and interview with staff, the center failed to ensure that staff -to- child ratios were maintained.
Evidence:
1. From approximately 9:20 AM until 9:50 AM, the licensing inspector observed 14 children (ranging from age 13 months to 4 years) with 2 staff in the main play area. The required ratio is 4:1.
2. The Director acknowledge that the group was out of ratio.

Standard #: 8VAC20-780-500-A
Description: (Repeat Violation) Based on observation, the center did not ensure that staff and children?s hands shall be washed with soap and running water.

Evidence:
1. Staff #1 was observed changing a child's diaper. After the diaper change the child was placed back in the infant/toddler area and began playing with toys. Staff did not wash the child?s hands.
2. The children finished eating breakfast and began the next activity without washing their hands with soap and running water or disposable wipes.
3. Staff did not was their hands before or after serving the children breakfast.
