Comments:
An unannounced monitoring inspection was conducted on 1/31/24 from 4:00pm to 5:05pm with program staff. There were 34 children in care, ranging in age from five- to 12-years-old, supervised by two staff. The children were observed transitioning from the bathroom, having free play, playing a group game, and going back to the bathroom to wash hands for snack. Four child records and three staff records were reviewed. The Center had two staff present with current certification in CPR and First Aid, and one staff trained in Daily Health Observation Training. Three medications and authorization forms were reviewed and the center had one staff present who was current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, allergy list, and emergency drill log were requested. Snack served was cheese sticks and apples. Please complete the "Plan of Correction" and "Date to be Corrected" areas on the Violation Notice for each violation cited and return to me by close of business on 2/14/24. Plans of correction should include steps to correct the noncompliance with the standard, and measures to prevent the noncompliance from occurring again.

Violations:
Standard #: 22.1-289.035-A
Description: Based on review of three staff records and interview, the center did not ensure that the results of a central registry check were obtained for staff every five years from the date of the last check. Evidence: The most recent central registry check results on record for Staff #3 were dated 12/20/2018.

Plan of Correction: The FCCC main office will alter and adjust procedures around ensuring that all checks are completed prior to expiring.

Standard #: 8VAC20-780-40-M
Description: Based on interview, the center did not maintain, in a way that was accessible to all staff who work with children, a current written list of all children?s allergies, sensitivities, and dietary restrictions. Evidence: The allergy list was requested from staff on 1/31/24 and staff reported that an allergy list did not exist for the program.

Plan of Correction: FCCC maintains allergy lists in readily accessible locations consistent through each of our centers. The site staff will be retrained on FCCC's procedures and the location where such records are kept in every site and location.

Standard #: 8VAC20-780-550-G
Description: (REPEAT VIOLATION) Based on review of documentation, the center did not maintain documentation of emergency drills. Evidence: The most recent evacuation drill documented on the drill log was conducted in September 2023. There was no documentation of drills for October, November, December, or January.

Plan of Correction: The Site Manager in charge of the location will be retrained on licensing regulations, specifically the required documentation around emergency drills.
