Comments:
An unannounced monitoring inspection was conducted from 1:00 pm - 2:45 pm. There were 76 children in care being supervised by 7 staff. The children were observed resting. . The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and six staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-280-B
Description: Based on observation of a classroom known as "The Brooke" it was determined that all hazardous substances were not kept in a locked place preventing access by children as required. Evidence: A spray bottle of disinfectant was observed on top a low refrigerator and not locked as required. The children being cared for in "The Brooke" ranged in age from infants - toddlers.

Standard #: 8VAC20-780-350-B-1
Description: Based on observation of a combined classroom caring for infants and toddlers it was determined that a staff to child ratio of 1:4 was not always followed. Evidence: On 01/25/2024 at approximately 1:10 pm the LI (Licensing Inspector) observed a classroom of combined infants and toddlers. The room was combined due to a malfunction with the facilities water and therefore these classrooms were displaced and combined in a larger room known as "The Brooke." The room was divided with 4 infants on one side being supervised by 1 staff. The other half of the room there was one staff caring for 10 children, mostly toddlers who were resting. The age of the youngest child (Child B) in the toddler group was 15 months, therefore a ratio of 1:4 should have been followed instead of a resting ratio for toddlers which is 1:10.

Description: Based on review of medication it was determined that not all of the center's procedures were followed in regards to administering medication, ensuring methods to prevent the use of outdated medication. Evidence: The emergency medication on site for child A had an expiration date of November 2023.

Description: Based on review of two medications, the facility failed to notify parents that that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed. Evidence: The authorization forms for two emergency medications for Child A expired on September 30, 2023. The authorization was not renewed and the medication was not destroyed after 14 days as required.
