Comments:
An unannounced monitoring inspection was conducted on February 8, 2024, from 10:05am-12:00am. At the time of entrance, ( 61 ) children were in care with ( 11 ) staff members present. The inspector reviewed 3 children?s records and 3 staff records. Children were observed during snack time, engaged in individual activities to include name tracing, matching, number line, Chinese Zodiac coloring and dance class. Medications were observed. Areas of non-compliance are identified on the violation notice. Repeat violations were found and are identified within this report.

Violations:
Standard #: 8VAC20-780-510-I
Description: Repeat Violation
Based on observation and staff interview, the center has not obtained written authorization from a parent to administer a medication and the center has not maintained an original labeled container with the prescription label for all medications.
Evidence: Child #4 has a Auvi Q medication at the center to be administered in an emergency, there is no parent or physician authorization to allow administration of this medication.
Child #5 was given Albuterol Sulfate by a nebulizer at 11:30 am. The medication was not in a labeled container with the pharmacy label attach. The vial was found loose in a bag with no name on the vial.

Plan of Correction: The parent will be notified that the medication must be in the original box with the prescription label attached.
The parent will complete the medication authorization form to allow the Auvi Q to be administered in an emergency.
