Physical plant issues with the roof that had been leaking since 9/26/23.

Comments:
An investigation was begun on 1/9/2024 in response to a self reported incident (child's injury) being received in our office on 1/3/24. During the course of the investigation, an additional report was received regarding a child's allergic reaction.
A previously reported issue (potential mold) was also investigated during this inspection.

1. Based on multiple statements, the preponderance of evidence does not support any violation regarding the care of children.
2. Based on multiple staff statements, the preponderance of evidence does support the allegation that a child was given an allergen and had an allergic reaction.
3. Based on observation and staff statements, the preponderance of evidence does support a physical plant violation regarding water damage to the facility.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center was not maintained in a clean, safe and operable condition.
Evidence:
1. The ceiling in the lobby/reception area of the facility had evidence of water damage. The damage included large water stains with peeling paint and plaster next to the track lighting fixture.
2. The ceiling in the Teacher's lounge area had an active water leak. A tarp and trash cans were in place to catch water dripping from the ceiling. Multiple ceiling tiles had large water stains visible.
3. Multiple ceiling tiles in Room 4 had evidence of water damage, Tiles next to the air exchange grate and the heat vent had large brown stains visible.
4. Multiple ceiling tiles in Room 7 had evidence of water damage. Tiles along the left wall upon entering the room, had multiple large brown stains visible. One ceiling tile (approximately 18" x 24") is missing. Staff stated the tile fell to the floor after being saturated with water.

Plan of Correction: We are currently collaborating with a reputable roofing company and remediation company to repair the roof damage caused by water and to remove and replace all ceiling tiles affected by stains in the lobby area, staff lounge area, Room 4, and Room 7. The repairs and remediation work are scheduled during the first week of March, 2024. This process will involve the removal and replacement of damaged ceiling tiles and roof areas, to prevent further water damage. We have proactively communicated with both parents and staff members of the upcoming roof repairs and remediation. The forthcoming repairs and remediation efforts are aimed at ensuring the school premises are maintained in a clean, safe, and fully operable conditions.

Description: Based on multiple staff statements, the center failed to provide care, protection and guidance to a child in care.
Evidence:
On January 8, 2024 a child with a diagnosed and documented food allergy, was given a allergen for snack. The child began experiencing an allergic reaction on the way home, vomiting in the car and at home. (Child #2)

Plan of Correction: We are currently conducting an audit of all students with known allergies. This includes deciphering whether the allergy is a contact allergy or ingestion allergy. We updated the allergy lists in all classrooms to ensure they accurately reflect the allergies of each student. We are organizing a training session on health and safety for all staff members. This training will cover proper allergy management protocols, including prevention of cross-contamination and appropriate handling of allergens, as well as avoiding serving known allergens to students with allergies.
