Comments:
An unannounced monitoring inspection was conducted on 2/1/2024. There were 61 children, ages 4 months -4 years, 15 staff and 2 administrative staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were observed. A total of 5 children?s records and 3 staff records were reviewed. The children were observed during breakfast, center time and circle time. The inspector arrived for the inspection at 8:30am and departed the center at 10:50am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Violations:
Standard #: 8VAC20-780-240-B
Description: Based on the review of three staff records, the provider failed to ensure that staff shall complete orientation training prior to the staff member working alone with children and no later than seven days of the date of assuming job responsibilities.

Evidence: Staff #2 start date was 10/23/2023 and on the day of the inspection did not have staff orientation completed.

Plan of Correction: Provider will ensure staff receive orientation training.

Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas and equipment shall be maintained in a clean, safe and operable condition.

Evidence:
1. The toilet tank lid in the Toddler room bathroom was broken on the back left corner and sharp edges were exposed. No children were observed in the area but it was accessible to children.

2. The brown plastic storage box by the fence closest to the parking lot on the small playground is broken and has a metal bracket at the front and rear right corners exposed with sharp edges. No children were observed in the area but it was accessible to children.

Plan of Correction: Provider will repair toilet tank lid and remove broken storage container from play ground.

Standard #: 8VAC20-780-380-A
Description: Based on observation and discussion with staff, the center failed to ensure that there shall be a posted daily schedule that allows for flexibility as children?s needs require.

Evidence: Licensing staff observed that no daily schedule was posted in the Two?s room located next to the large playground. Staff stated that ?they are not normally in that room and were not sure where the schedule was at?.

Plan of Correction: Provider will post daily schedule in classroom.

Standard #: 8VAC20-780-500-B
Description: REPEAT VIOLATION

Based on observation, the center failed to ensure that the required nonabsorbent surface for diapering was used.

Evidence: The changing pad in the Waddler?s classroom had a tear in the material approximately 2 inches long in the middle and the padding was exposed.

Plan of Correction: Provider will replace changing pad.
