On 01/11/2024 from 12:30 PM to 1:30 PM and 01/25/2024 from 12:30 PM to 1:30 PM an unannounced inspection was conducted in response to a self-reported incident received on 12/18/2023. Interviews were conducted regarding the incident and records and policies and procedures were reviewed. The information gathered during the inspection resulted in violations. Areas of non-compliance are identified on this Violation Notice.

Evidence:
Per interview with adult 1 and staff 2: In the Early 2?s classroom on 12/18/2023, while staff 1 was changing diapers and later, on the other side of the room setting up cots, Staff 2 fell asleep twice for a total of approximately 4 ? minutes.Plan of Correction: SEE INTENSIVE PLAN OF CORRECTION
