Technical Assistance:
Provided technical assistance on standard:
1) 40N: the center shall develop written playground safety procedures that shall include provision for active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children.
2) 350F: the center shall develop and implement a written policy and procedure that describes how the center will ensure that each group of children receives care by consistent staff or team of staff members.
3) 500B: the diapering area shall have a leakproof, covered receptacle for soiled linens
4) 540C: the first aid kit shall have single use gloves
5) 550B: the emergency plan shall have procedural components including, "procedures to reunite children with a parent or authorized person designated by the parent to pick up the child."
6) 570I: A one-day's emergency supply of disposable bottles, nipples, and commercial formulas appropriate for the children in care shall be maintained at the center.

Comments:
An announced initial inspection was conducted from 11:15 am - 3:45 pm with management staff, corporate staff and the owner. No children were present during this inspection. A qualified director is in place and was present during the inspection. All 10 classrooms and the multipurpose room were observed and measured for capacity. The center is well-equipped with age and stage appropriate furnishings, equipment, and materials. The 24 cribs meet Consumer Product Safety Commission (CPSC) federal safety standards for cribs (manufactured 2022). There are sufficient bathrooms, sinks, and adult bathrooms. Four staff/applicant records were reviewed. The center's policies and procedures, emergency supplies, and menu were reviewed and discussed. Three playground areas were observed and measured. Standard #: 22.1-289.035-B-2
Description: Based on review of documentation, it was determined that not all staff had an Office of Background Investigations (OBI) fingerprinting-based criminal history record check prior to employment, as required. Evidence: the record for staff #3 (date of hire: 11/27/2023) had an OBI determination letter dated: 1/12/2024.

Plan of Correction: Going forward, will ensure timelines are followed for all background checks.

Standard #: 8VAC20-770-60-C-2
Description: Based on review of documentation, it was determined that not all staff had Central Registry Search results within 30 days of hire, or documentation that follow up was completed as required. Evidence: the record for staff #3 (date of hire: 11/27/2023) had Central Registry search results dated: 1/17/2024.

Plan of Correction: Going forward, will ensure timelines are followed for all background checks.

Standard #: 8VAC20-780-160-A
Description: Based on review of documentation, not all staff had documentation of a negative Tuberculosis (TB) test/screening at the time of employment. Evidence: The record for staff #3 (date of hire: 11/27/23) had documentation of a negative TB test/screening dated: 1/22/2024.

Plan of Correction: Going forward, will ensure timelines are followed for TB tests/screenings.
