Comments:
A monitoring investigation was initiated on February 7, 2024 regarding a self-reported incident received by the Fairfax Licensing Office on February 6, 2024. Video footage was reviewed and interviews were conducted. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-240-D
Description: Based on review of staff records, prior to working alone with children and within seven days of the first day of employment, staff was not provided in writing the centers procedures for identifying where attending children at all times.
Evidence
Staff records did not contain documentation of receiving in writing the centers procedures for identifying where attending children are at all times.

Plan of Correction: The employee handbook will be updated to include our child supervision policies and employees will sign acknowledgment of the policies. This will be completed no later than February 29, 2024.

Standard #: 8VAC20-780-340-A
Description: REPEAT VIOLATION
Based on review of video footage and staff interviews, when staff are supervising children, they did not always ensure their care protection and guidance.
Evidence
Staff #1, #2 and #3 did not always ensure the care, protection and guidance of child #1(three years old).
1.On February 6, 2024 at approximately 10:30am staff #1, #2 and #3 lined the children up in the CASA 1 classroom to go outside.
2. At approximately 10:31am the three staff members and children left the CASA 1 classroom. Closing the classroom door after they excited.
3 At approximately 10:32am Child #1 is observed coming out of the bathroom in the CASA 1 classroom.
4. At 10:34am child #1 is observed opening the door of the CASA 1 classroom and exciting into the hallway.
5. Child #1 is observed walking back and forth in the hallway past the CASA 1 classroom.
6. At approximately 10:35am child #1 is approached in the hallway by a staff member and taken to the front office.
7. Child #1 was not under the care, protection and guidance of a staff member for approximately four minutes.

Plan of Correction: Retrain staff to count children every time they leave the classroom, to check the bathrooms, recount when they exit the locked door into the lobby ,before leaving the building and recount when they arrive at the playground. They will count the children before they leave the playground and recount when they arrive back at the lobby before entering the locked door into the hallway.

Evidence
