Comments:

A monitoring inspection was conducted on 02/07/2024. There were a total of 48 children with 7 staff members present. Children were observed during lunch, on the playground, and playing in learning centers. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 4 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today in order for it to be included on the website. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Standard #:	22.1-289.035-B-2
Description:	Based on a review of staff records, the center did not have criminal history results for each staff member prior to working with children. Evidence: Staff #1 (date of hire 2/6/2024) did not have criminal history results on record during the inspection.
Plan of Correction:	She did not have an appointment until 2/10/2024.
Standard #:	8VAC20-770-60-C-2
Description:	Based on a review of staff records, the center did not have central registry results for each staff member within 30 days of employment. Evidence: Staff #3 (date of hire 8/21/2023) and staff #4 (date of hire 10/9/2023) did not have VA central registry results within 30 days of date of employment.
Plan of Correction:	We sent the 1st one the copy of it was in the file. We never checked to make sure it came back. We called and sent an email. It came back cleared.
Standard #:	8VAC20-780-160-A
Description:	REPEAT VIOLATION Based on a review of staff records, the center did not ensure that each staff member submitted documentation of a negative tuberculosis screening prior to coming into contact with children. Evidence: Staff #1 (date of hire 2/6/2024) and staff #4 (date of hire 10/9/2023) did not have a TB test or screening on record during the inspection.
Plan of Correction:	The staff hired on 2/6/24 did not have an appointment until 2/12/2024. It came back cleared. The second staff took a screening. We will make sure the files are cleared.
Standard #:	8VAC20-780-70
Description:	Based on a review of staff records, the center did not ensure all required information was available. Evidence: Staff #1 (date of hire 2/6/2024), staff #3 (date of hire 8/21/2023), staff #4 (date of hire 10/9/2023) did not have an emergency contact documented.
Plan of Correction:	All files have been placed in specific notebooks for every teacher so that they are easy to locate.
Standard #:	8VAC20-780-245-A
Description:	Based on a review of staff records, the center did not ensure each staff member had documentation of orientation. Evidence: Staff #4 (date of hire 10/9/2023) did not have documentation of orientation on record within 7 days of hire.
Plan of Correction:	The orientation papers will be placed in front that it is easy to assess opposed to the back with emergency contacts.
Standard #:	8VAC20-780-350-B-3
Description:	Based on observation, the center did not ensure required ratios were maintained whenever children are in care. Evidence: The 2 year old classroom (Rising Rainbows) was observed on 02/07/2024 with one teacher and eleven children from approximately 11:10 am until approximately 11:40 am.
Plan of Correction:	The Lead Teacher got sick and the Substitute came at 11:40 to help her. We hired an agency to help with substitutes.
Standard #:	8VAC20-780-570-A
Description:	Based on observation, the center did not ensure protective belts on highchair tables were fastened securely. Evidence: During today?s inspection, 6 of 6 children that were sitting in the highchair table did not have the protective belt fastened.
Plan of Correction:	The teachers were reminded to lock the children in as soon as they are put in the chairs.
