Comments:
A renewal inspection was conducted on February 6, 2024 with the center director and staff. There were 39 School age children present, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records were reviewed. Two staff files were sent electronically on 2/09/2024.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-130-A
Description: Based on review of six children?s records, the facility failed to have the immunization record for each child before the child can attend the center. Evidence: Child 1 (date of enrollment 1/08/2024) and Child 3 (date of enrollment 1/08/2024) did not have documentation of an immunization record on file.

Plan of Correction: Staff will request this information from the parent or the school nurse.

Standard #: 8VAC20-780-140-A
Description: Based on review of six children?s records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 1 (date of enrollment 1/08/2024) and Child 3 (date of enrollment 1/08/2024) did not have documentation of a physical. on file.

Plan of Correction: Staff will request the information from the parent or the school nurse.

Standard #: 8VAC20-780-60-A
Description: Based on review of six children's records, the facility failed to have a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child A and Child B have diagnosed food allergies but do not have a written allergy action plan on file.

Plan of Correction: Staff will request this information from the parent or the school nurse.

Standard #: 8VAC20-780-550-F
Description: Based on review of the emergency drill log, the facility failed to conduct a lockdown drill at least once annually. Evidence: there was no documentation of a lockdown drill. since October 2022.

Plan of Correction: A lockdown drill will be conducted this month.
