Technical Assistance:
Discussed the best practices to ensure that bathroom toilets and floors are maintained and cleaned properly.

Comments:
An unannounced monitoring inspection was conducted on February 13, 2024 between the hours of 9:10am and 1:00pm. There were a total of 58 children with 12 staff members supervising. Children were observed engaged in small group activities, painting, sensory table, art, various age appropriate toys, toileting, handwashing, and naptime. Positive interaction was observed during the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, children and staff records, special care and emergencies and nutrition. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility. Violations:
Standard #: 8VAC20-780-160-A
Description: Based on record review, the documentation of a negative tuberculosis (TB) screening was not submitted at the time of employment and prior to coming into contact with children.
Evidence:
1. The date of hire for Staff #2 was 07/24/23 and the documentation for a negative TB screening was dated 09/18/23.

Repeat violation: Previous violation 01/03/23.

Plan of Correction: Going forward, I will not allow employees to start until I have the current TB results.

Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the Virginia Department of Education-sponsored orientation course was not completed within 90 calendar days of employment.
Evidence:
1. At the time of inspection (2/13/24) , Staff #5 has not completed the orientation course and the date of hire for Staff #5 was 09/18/23.
2. The center director confirmed the orientation course has not been completed at this time.

Plan of Correction: Employee will complete the course by this Friday. I will ensure any new employees complete the course within 90 days of employment.

Standard #: 8VAC20-780-280-B
Description: Based on observation, hazardous substances, such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. The Two's classroom had a bottle of bleach water, a bottle of detergent and a bottle of soap water on the bottom shelf inside an unlocked changing table.

Repeat violation: Previous violation on 01/03/23 and 07/12/23.

Plan of Correction: I reviewed policy with teachers and will monitor the teachers to ensure they are relocking the cabinet doors after cleaning the diapering area.

Violation was corrected during the inspection.

Standard #: 8VAC20-780-280-D
Description: Based on observation, cleaning and sanitizing materials were located above utensils and single-service articles.
Evidence:
1. Cleaning and sanitizing materials (bottles of bleach water, soap and detergent) were stored inside a changing table above single-use articles (paper plates, bowls, plastic drinking cups and utensils) in the Two's classroom.

Plan of Correction: I spoke to the teachers and the items were moved to a different location away from cleaning and sanitizing materials. I will review again at our staff in-service training next week.

Violation was corrected during inspection.

Standard #: 8VAC20-780-520-C
Description: Based on observation, diaper ointments were not labeled with a child's name and products were accessible to children.
Evidence:
1. There were 4 diaper ointments (Cerave, 2 tubes of Destin, and Butt Paste) not labeled with a child's name in the Infant-B classroom.
2. Various diaper ointments and creams stored inside the changing table were accessible to children in the Two-year-old classroom.

Repeat Violation: previous violation 07/12/23

Plan of Correction: I will speak to my staff and the names will be written on the ointment immediately. Also, I will retrain the staff at our in-service training next week.
Violation was corrected during the inspection.

Standard #: 8VAC20-780-560-G
Description: Based on observation and interview, food container brought from home was not dated and labeled in a way that identifies the owner.
Evidence:
1. A bottle of milk in the Infant-B classroom was not labeled.
2. Staff #6 acknowledged the bottle was not labeled and belong to Child #6.

Plan of Correction: I have spoken to the teacher and the bottles have been labeled.
Violation was corrected during the inspection.
