An unannounced inspection was conducted on 12/01/2023 from approximately 10:10 a.m. ? 4:00 p.m. The inspector reviewed the areas of noncompliance identified in the Intensive Plan of Correction (IPOC). The information gathered during this inspection determined continued noncompliance of the applicable standards. Based on observation the center failed to ensure that children under 10 years of age always shall be within actual sight and sound supervision of staff.
Evidence:
A child, age 3, was observed by licensing inspector walking down the hallway by himself. He entered a hallway bathroom without staff providing actual sight and sound supervision.
