The following was discussed with the program director and assistant director:
8VAC20-780-60. Children's records - please review all required information that is to be documented in each enrolled child's file.
Supervision - It is important that staff are positioned in a way that they are able to provide sight and sound supervision at all times.
- Comments:

A renewal inspection was conducted on 02/13/2024. A tour of the facility was conducted, in addition to a record review and interviews with staff. The Inspector reviewed both children and staff records, along with any requested program records submitted by the facility to determine if required documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-270-A Description: Based on observation the center did not ensure all areas and equipment of the center were maintained in a safe condition.
Evidence:
In the twos bathroom the coating of 2 toilet seats were peeling. A third toilet had a cover that did not fit the tank. It was too big and unstable and was observed duct taped to the tank.Plan of Correction: Work orders had been previously placed for the toilet seats. The director will follow-up on the status. A work order will be placed to replace the toilet tank cover with one that fits securely.
Standard #: 8VAC20-780-540-A Description: Based on observation the center did not ensure all required items were in the first aid kit.
Evidence:
The first aid did not have a working thermometer or a first aid manual.Plan of Correction: An audit of the center's first aid and emergency supplies had been done and an order has already been placed for needed supplies. As soon as they arrive all first aid kits and emergency supplies will be complete.
Standard #: 8VAC20-780-540-E Description: Based on observation the center did not have all nonmedical emergency supplies.

The center did not have a working, battery-operated radio.Plan of Correction: An audit of the center's first aid and emergency supplies had been done and an order has already been placed for needed supplies. As soon as they arrive all first aid kits and emergency supplies will be complete.
