Comments:
An unannounced monitoring inspection was conducted on February 6, 2024 from approximately 12:00 pm to 1:30 pm. There were 10 children in attendance and a total of 4 staff present. 3 staff records and 6 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed taking a nap while the licensing inspector was on-site.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 02/15/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center failed to ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence:
1. The wooden fence surrounding the playground had several rough, splintered, cracked, and areas of deteriorating wood that could snag skin.
2. Review of injury reports revealed that several children have gotten splinters in their skin as a result of the rough wooden fence.

Plan of Correction: Director contacted Board chairman on 02/15. They will schedule maintenance on the fence to be done by 03/01. The Director will continue to monitor condition of fence and reach out for repair as needed.

Standard #: 8VAC20-780-330-B
Description: Repeat Violation
Based on observation, the center failed to ensure where playground equipment is provided, resilient surfacing shall comply with minimum safety standards in the fall zones.
Evidence: The fall zones around the playground equipment had less than 6 inches of mulch. When mulch is used as a resilient surface in fall zones there must be 6 inches of mulch underneath and surrounding equipment up to 6 feet on all sides.

Plan of Correction: Director contacted Board chairman on 02/15 since they are in charge of ordering mulch. They will have mulch delivered by 03/01. The Director will continue to rake and monitor mulch levels and ask for more from the Board when needed.
