Comments:
An unannounced monitoring inspection was initiated on 2/15/24 at approximately 9:24am and concluded at approximately 11:00am. There were a total of 7 children with 2 staff members present. Children were observed during snack time, engaged in group and independent activities. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 children?s records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-40-M
Description: Based on observation and interview, the center did not maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan.
Evidence:
1. Record review documented a child with food allergy
2. Staff stated a current written list of all children's allergies, sensitivities, and dietary restrictions does not exist.

Plan of Correction: Written list of allergies, sensitivities and dietary restrictions will be posted accessible to all staff, in addition to the app.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center did not have documentation of viewing proof of the child's identity and age, within 7 days of attending.
Evidence: No date documenting when proof of identity was viewed for Child#1.

Plan of Correction: Documentation of viewing proof of child?s identity and age will be kept in child?s file.

Description: REPEAT Based on record review, the center did not have documentation that two or more references were checked before employment.
Evidence: 1. Staff #1 (start date 9/05/2023) did not have documentation of two or more references before employment.

Plan of Correction: Documentation that two or more references were checked will be placed in staff?s file.

Description: Based on record review and interview, a child prescribed emergency medications is not always in the care of a staff member who meets the requirements of administering emergency medications.
Evidence: Child#3 has prescribed emergency medications. No staff at the center have the required training.

Plan of Correction: All staff will get the required training.

Description: Based on observation and record review, the center did not have emergency evacuation and shelter-in-place procedures or maps posted in a location conspicuous to staff and children on each floor of the building.
Evidence:
1. Emergency evacuation and shelter-in-place procedures or maps were not posted on lower level, used by children.
2. Main level, used by children did not have Shelter in Place procedures or maps posted.

Description: Based on observation and interview, food brought from home did not have food container clearly dated and labeled in a way that identifies the owner.
Evidence: Two food containers examined were not dated.

Plan of Correction: Food containers brought from home will be clearly labeled with name and date daily.
