Comments:
A renewal inspection was completed on February 15, 2024 from 9:42 A.M.-12:00 P.M. There were 28 children present, ranging in ages from three to five years of age, with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-550-P
Description: Based on review of injury reports, the center failed to ensure all required information was completed and on the injury report.

Evidence:
An injury report dated 11/14/2023 was missing information how the parent was notified of the injury, and the future action to prevent reoccurrence was not completed.

Plan of Correction: Administration pulled the model injury report off the Department of Education website and will use this form for future injury reports.
