An unannounced 60-day inspection was conducted on February 15, 2024. The inspection began at approximately 10:00 a.m. and concluded at 11:00 a.m. There were 3 children present, ranging in ages from 11 months to 2 years, with 1 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies. Transportation is not provided. There were no medications on site. A total of 3 child records and 1 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-60-A Description: Based on review of 3 child records, the center failed to ensure that each child?s record contained the required information.
Evidence:
1. The record for child # 1, did not contain documentation of the parent?s work number and the parent's place of employment. Child #1?s date of enrollment was 1/22/2024.
2. The record for child #2, did not contain documentation of the parent?s work number, place of employment, and a 2nd emergency contact. Child # 2?s date of enrollment was 2/14/2024.
3. The record for child #3, did not contain documentation of the parent?s work number, parent's place of employment, and a 2nd emergency contact. Child #3?s date of enrollment was 2/14/2024.Plan of Correction: All files will be updated immediately for each child.
