Technical Assistance:
The facility's plans to upgrade the hallway carpeting, threshold to the lunch room and the boys bathroom were discussed.

Comments:
A monitoring inspection was initiated and concluded on February 13, 2924. The inspector arrived at 11:00 AM and departed at 1:15 PM. There were 8 children present with 2 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director during the exit interview.

Violations:
Standard #: 8VAC20-780-270-A
Description: Repeat violation.
Based upon observation, the facility has not ensured that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition.
Evidence:
1. The downspout from the gutters to the ground near the exit door used by the children to access the playground is missing. There is a large puddle of water four inches deep below where the downspout should have been.
2. The gutters on the back of the building are overflowing with leaves and debris.

Plan of Correction: The facility responded with the following:
The gutters will be cleaned out and a new downspout with be connected. The hole at the base of the downspout will be filled so water does not accumulate at the base.

Standard #: 8VAC20-780-280-B
Description: Based upon observation, the facility has not ensured that hazardous substances are kept in locked places using safe locking methods that prevent access by children.
Evidence:
The door to the hallway closet was not locked. Inside the closet were containers of bleach and disinfectant with warning labels stored on the floor.

Plan of Correction: The facility responded with the following:
A lock or child proof latch will be added to the hallway closet. All staff will be reminded that the closet must be locked at all times.

Standard #: 8VAC20-780-320-B
Description: Based upon observation, the facility has not ensured that each restroom is equipped with toilet tissue.
Evidence:
There were two stalls in the girls bathroom in the long hallway that did not have toilet tissue.

Plan of Correction: The facility responded with the following:
The toilet tissue was replaced by staff during the inspection.

Standard #: 8VAC20-780-500-B
Description: Based upon observation, the facility has not ensured that soiled diapers are disposed in a storage system that is either foot-operated or used in such a way that neither the staff member's hands nor the soiled diaper touches an exterior surface of the storage system.
Evidence:
1. Staff 4 was observed in the process of changing a child's diaper. The lid on the diaper disposal system was broken and could not be opened without using hands. Staff 1 opened the lid with her hands to find the storage system was stuffed full of soiled diapers. As the foot operation was not in working order, diapers had been previously disposed using the hands to operate the lid.

Plan of Correction: The facility responded with the following:
A new, larger, foot operated diaper disposal can will be purchased. Diaper changing procedures will be reviewed and enforced with all staff.
