Comments:
On 1/9/2024, an unannounced investigation was conducted regarding a self-reported incident involving care and protection of children was begun. This visit was conducted between 12:45pm-2:10pm. Interviews were conducted, documents were reviewed and observations were made. After carefully reviewing all information, violations were cited during this visit. Violations:
Standard #: 8VAC20-780-340-A
Description: Based on staff interview, when staff were supervising children, they did not always ensure their care and protection.

Evidence:
1. On January 4, 2024, at approximately 3:45pm, during afternoon pick-up time, Staff #1 (doh: 5/16/2013) in the Infant room give a bottle that contained approximately 6 ounces of a mixture of ? breast milk & ? formula to Infant #1. The bottle did not belong to Infant #1.
2. Staff #1 fed the whole bottle to Infant #1.
3. Staff #1 realized the bottle that was just fed to Infant #1 did not belong to Infant #1 at the time that Infant #1 was being picked up by their parent in the afternoon.
4. Staff #1 did not always ensure the care and protection of Infant #1.

Plan of Correction: Staff in the Infant room were retrained on the Infant Feeding Procedure and are now confirming with another staff member that the bottle matches the child that they are going to feed (verbal confirmation).
