Comments:
An unannounced inspection was conducted on 2/20/2024 in response to a self report that was received by the licensing department on 2/14/2024 regarding allegations in the areas of supervision and special feeding needs. On 2/20/2024, the inspector was on site from approximately 1:20 pm to 2:32 pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed.

The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the self report is determined to be valid. See the violation notice for more details.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word corrected is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.

Violations:
Standard #: 8VAC20-780-340-A
Description: REPEAT VIOLATION

Based on observation and interview, the center did not ensure that when staff are supervising children, they always ensure their care and protection.

Evidence:
1. On 2/14/2024, staff #1 (date of employment: 6/27/2022) gave child #1 (date of enrollment: 6/5/2023) a 9 oz bottle of formula. Staff # 2 reported she saw child #1's bottle unattended on the floor with approximately one ounce remaining. Staff #2 stated that she was busy at the time but when she looked back she saw child #2 (date of enrollment: 9/18/2023) drinking from the bottle. When the bottle was obtained from child #2 it was empty. Multiple staff reported that they believed child #2 consumed about an ounce of child #1's formula.
2. Administration acknowledged that staff #1 did not adequately supervise child #1 after giving the child the bottle which led to child #2 consuming unauthorized formula.

Plan of Correction: All staff will be retrained on feeding procedures and supervision during
feedings by 2/29/24

Standard #: 8VAC20-780-570-C
Description: Based on a review of documentation and interview, the center did not ensure that the record of each child on formula contained the brand of formula as required.

Evidence:
1. The record of child #2 (date of enrollment: 9/18/2023)did not contain the brand of formula.
2. Administration and staff acknowledged the information was not in the record.

Plan of Correction: A list of the children?s formula brands will be posted by 2/29/24
