Comments:
An unannounced renewal Inspection took place on February 15, 2024 between the hours of approximately 11:45 a.m. and 1:40 p.m. There were 3 classrooms observed with a total of 46 children with 6 staff within the supervision guidelines. The children were observed eating lunch, napping and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures was conducted during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description: Based on record review, the center did not obtain all information for 1 out of 3 staff records.

Evidence: Staff #1?s (start date: 11/29/2023) record was missing 1 out of 2 reference checks.

Standard #: 8VAC20-780-245-L
Description: Based on observation and record review, the center did not ensure that there was always at least one staff member on duty who has obtained within the last three years instruction in performing the daily health observation of children.

Evidence: At the start of the inspection, there were no staff on duty who had received Daily Health Observation training.

Description: REPEAT VIOLATION

Based on observation, hazardous substances were not kept in a locked place.

Evidence: 1 all-purpose cleaner, 1 disinfectant spray, and 1 air freshener were in an unlocked cabinet in the Primary classroom, accessible to children.

Description: Based on observation, the center did not ensure that rest mats were identified for use by a specific child.

Evidence:
1. Staff #4 was observed removing rest mats from a pile, putting sheets on the mats and placing them on the floor for the children to nap on.
2. Child #1, Child #6 and Child #7 were observed lying on the mats.
3. The mats were not identified for use by a specific child.

Plan of Correction: Numbered all mats

Description: Based on observation, the center did not ensure that there was at least 12 inches of space between occupied rest mats.

Evidence:
1. There was approximately 3 to 4 inches between rest mats occupied by Child #1 and Child #6.
2. There was approximately 3 to 4 inches between rest mats occupied by Child #1 and Child #7.

Plan of Correction: Remind staff about spacing

Description: Based on observation, the center did not ensure that children?s hands were washed with soap and running water after eating meals.

Evidence: Child #1 and Child #7 were observed finishing their meals and then going to their rest mats without washing their hands.

Description: Based on observation, the center did not ensure that a diapering surface was used only for diapering or cleaning children.

Evidence: There was artwork, a box, a bin with puzzle pieces and a roll of paper towels covering a diapering surface in the Toddler classroom.

Based on observation, the center did not ensure that diaper creams were inaccessible to children.

Evidence: There were 5 diaper creams located in bins in the Toddler classroom, accessible to children.

Plan of Correction: Removed all ointment

Description: REPEAT VIOLATION

Based on observation, the center did not ensure that food containers were clearly dated and labeled in a way that identifies the owner when food is brought home.

Evidence: 3 out of 3 lunchboxes were not dated and labeled with the owners? names.

Plan of Correction: Dated all lunch containers
