Technical Assistance:
8VAC20-780-70 Discussion regarding staff records to include date of employment or volunteering and job title.

Comments:
An unannounced monitoring inspection was conducted on 01/31/2024 from 9:20 a.m. to 12:20 p.m. with the Director. There were 52 children in care, ranging in age from three months to six-years-old, supervised by 12 staff. The children were observed in circle time, eating morning snack and free play. Five child records and five staff records were reviewed. Three medications and authorization forms were reviewed, and the center has two staff current in Medication Administration Training (MAT). Required postings were observed. The attendance, menu, and emergency drill log were reviewed. The first aid kit, flashlight, and battery-operated radio were observed. The most recent Fire Inspection on file was dated 09/15/22 and the most recent Health Inspection on file was dated 01/18/23. Areas if non-compliance are identified in the Violation Notice. Standard #: 8VAC20-770-60-C-2
Description: Repeat Violation

Based on review five staff records, the center did not obtain documentation of the results of a central registry search within 30 days of employment for all staff members.

Evidence: The record for Staff #2, with a start date of 11/07/23 and Staff #3, with a start date of 12/04/23, did not contain documentation of the results of the Central registry background check at the time of inspection.

Description: Repeat Violation

Based on a review of five staff files, the center did not obtain documentation of a tuberculosis (TB) screening for staff at the time of employment and prior to coming in contact with children.

Evidence: The record for Staff #3, with a start date of 12/04/23, and Staff #5, with a start date of 09/08/23, did not contain documentation of a tuberculosis screening at the time of inspection.

Based on observation and interview, the center did not ensure they maintained a written record of daily attendance for each group of children documenting arrival and departure of each child in care as it occurred.

Evidence: In the Songbirds class, ages four-years-old, there were eight children present, the written record reflected there were six.

Description: Repeat Violation

Based on observation, it was determined that not all areas of the center inside and outside were maintained in a clean, safe, and operable condition.

1. In the Noah?s Ark class, ages 6 weeks to 16 months, a five-inch by five-inch piece of wood had broken off the ledge located next to the mini fridge in the classroom. The wood had sharp edges posing an impalement and scratch hazard.

2. In the Fruit of the Spirit II class, ages two to three-years-old, a rusted paper towel dispenser was observed over the children?s handwashing sink.

3. In the Overflow class, ages three-years-old, a metal file cabinet with chipping rust was observed.

4. In the Mighty Warriors class, ages five ? 12-years-old, the carpet in front of the handwashing sink was curling posing a trip hazard.

5. On the older playground two wooden boards, approximately 18? in height with semicircle cut outs at the top, from a previous water table that has been removed, protruding from the ground. The wood had two peaks on each board from the cut-outs, the wood was splintering and disintegrating, creating an impalement and scratch hazard. An enclosed wooden structure, used to store water hoses, had approximately six rusted nails protruding, and a rusted metal hinge, approximately 1.5? length, sticking out of the base of the structure creating an impalement and scratch hazard.

Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children.

Evidence: In the Fruit of the Spirit I class, ages 24-36 months, an unlocked cabinet was observed over the sink, the cabinet contained disinfectant wipes and toilet bowl cleaner.

Standard #: 8VAC20-780-330-B
Description: Repeat Violation

Based on observation, it was determined that there was insufficient resilient surfacing underneath and surrounding play equipment.

Evidence: On the 3/4/5/Schoolage playground the composite play structure had less than two inches of resilient surfacing underneath and six feet surrounding the structure. The requirement is six inches of resilient surfacing underneath and around this equipment.

Standard #: 8VAC20-780-500-B
Description: Based on observation, the center did not ensure that the diapering area contained a nonabsorbent surface for diapering or changing.

Evidence: In the Noah?s Ark class, ages 6weeks to 16 months; Gift of the Spirit class, ages 16-24 months; and the Fruit of the Spirit III class, ages 24-36 months, the diaper changing pads observed had several tears, making them absorbent.

Plan of Correction: Order changing pads. They have been replaced.
