A Subsidy Health and Safety inspection was conducted on 2/21/2024 to review supplemental health and safety requirements. There were 12 children present with 5 staff supervising.
A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 8VAC20-790-600-D-1 Description: REPEAT VIOLATION
Based on a review of five staff records and interview, the vendor did not ensure that one staff had current certification in cardiopulmonary resuscitation (CPR) appropriate to the ages of children in care.
Evidence:
1. The record of staff #5 contained a CPR certification dated 6/12/2021.
2. The vendor acknowledged that the certification was expired.Plan of Correction: The director will make sure all staff maintain a current CPR certification.
Standard #: 8VAC20-790-600-D-2 Description: REPEAT VIOLATION
Based on a review of five staff records and interview, the vendor did not ensure that one staff had current certification in first aid appropriate to the ages of children in care.

1. The record of staff #5 contained a first aid certification dated 6/12/2021.
2. The vendor acknowledged that the certification was expired.Plan of Correction: The director will make sure all staff maintain a current CPR/First Aid certification.
Standard #: 8VAC20-790-600-F Description: REPEAT VIOLATION
Based on a review of five (5) staff records and interview, the vendor did not ensure that one staff annually completed the department's health and safety update course as required.
Evidence:
1. The record of staff #5 contained a health and safety update certificate dated 2/15/2023. The staff was required to complete the update by 2/15/2024.
2. The vendor acknowledged that the course had not been completed annually as required.Plan of Correction: The director will ensure that all staff maintain a current health and safety and done annually.
