Comments:
An unannounced monitoring inspection was conducted today from 10:45am-2pm. There were 17 children directly supervised by 4 staff. The physical plant, 4 staff records, 3 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed playing outside. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.035-A
Description: Based on record review, fingerprint-based criminal history record check results were not obtained every 5 years for 1 of 4 staff reviewed.

Evidence:
1. The fingerprint-based criminal history record check results in the record for Staff #4 were dated 9/25/2023. The preceding fingerprint-based criminal history record check results in the record for Staff #4 were dated 7/23/2018, more than 5 years prior to the most current results.

Plan of Correction: The required background check results were obtained and filed. The Center will ensure repeat background checks are obtained prior to their expiration.

Standard #: 8VAC20-780-240-E
Description: Based on record review, 3 of 4 staff reviewed did not complete orientation in first aid and cardiopulmonary resuscitation (CPR) within the first 30 days of employment.

Evidence:
1. Staff #1 began employment on 10/02/2023. Staff completed orientation training in first aid and CPR on 1/16/2024, more than 30 days after the first day of employment.
2. Staff #2 began employment on 6/22/2023. Staff completed orientation training in first aid and CPR on 08/22/2023, more than 30 days after the first day of employment.
3. Staff #3 began employment on 11/06/2023. Staff completed orientation training in first aid and CPR on 1/16/2024, more than 30 days after the first day of employment.

Plan of Correction: The center now has more flexibility regarding staff availability and will schedule CPR and first aid training within required time frames.

Standard #: 8VAC20-780-270-A
Description: Based on observation, areas of the center outside were not maintained in a safe condition.

Evidence:
1. Two large detached tree limbs, each more than 6 feet in height, were observed leaning against a protruding limb of a tree in the outside play area. The limbs were not secure and could be easily moved by touching them.

Plan of Correction: The tree limbs were removed from the area during inspection.

Standard #: 8VAC20-780-550-B
Description: Based on record review, the center's emergency preparedness plan did not contain all required procedural components.

Evidence:
1. The most recent emergency preparedness plan does not contain procedures for lockdown.

Plan of Correction: The center began the process of gathering information to help develop procedures that best relate to their type of program. Procedures will be developed and included in the emergency preparedness plan.
