An unannounced monitoring inspection was conducted on 2/20/2024 at 12:20pm. There were 17 preschoolers directly supervised by 4 staff. The physical plant, 2 staff training records, 5 children?s records, no children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch, washing hands, preparing for lunch and socializing. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 8VAC20-780-60-A Description: Based on review, the facility did not ensure that children's records were maintained with the required minimum information.
Evidence:
1) reviewed 5 children's records and found that the facility did not have documentation of annually reviewed/updated emergency contact information: Child 1's last update was 5/11/22, Child 2's last update was 7/29/22, Child 4's last update was 2/9/2023, and Child 5's update was 8/15/22.
