Technical Assistance:
1. Reviewed annual review requirement and documentation of such for the emergency preparedness plan.
2. Updated background check information provided.

Comments:
Conducted an unannounced monitoring inspection at 9:45am. Observed 122 children + 21 direct-care staff. Ratios and supervision were in compliance. Children were observed in a variety of activities to include eating a morning snack, reading books, playing with toys, making an art project, and having active outdoor play. Staff were actively engaged with the children and activities. The program areas were found to be clean and well supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Questions about this inspection may be directed to pamela.sneed@doe.virginia.gov

Violations:
Standard #: 8VAC20-780-40-M
Description: Based on observations made, a list of child allergies was not kept in a confidential manner in one classroom. Evidence: The list was posted on a bulletin board and was visible to anyone that walked into the room.

Plan of Correction: Corrected during inspection. Added cover sheet for confidentiality.

Standard #: 8VAC20-780-270-A
Description: Based on observations made, an area in one classroom was not maintained in a safe condition. Evidence: An area on the wall in a toddler classroom had peeling and chipping paint. The location was at the children's level and was approximately 12" in length.

Plan of Correction: The toddler classroom was painted on 1/6/24. An area of the wall was damaged by a chair. Painters came back on 2/24/24 to fix the area.

Standard #: 8VAC20-780-510-F
Description: Based on records reviewed, a medication authorization form was not available to staff, though the medication was on-site. Evidence: There was no authorization form for a medicinal cream belonging to Child #13. 1

Plan of Correction: Sent home medicated diaper cream. Medicated cream is no longer on-site. Advised parent to check medicated creams to management. [sic]

Standard #: 8VAC20-780-510-G
Description: Based on observations made, a medical cream product was not labeled properly. Evidence: A medicinal cream was not labeled with the child's name, the dosage amount and the times to administer.

Plan of Correction: Re-trained staff on labeling all diaper cream ointments. All medicated cream will be checked in with management. Sent home medicated cream on 2/22/24.

Standard #: 8VAC20-780-520-C
Description: Based on observations made, diaper creams were not labeled properly in one classroom. Evidence: 2 diaper lotion/cream products were not labeled with child's name.

Plan of Correction: Corrected during inspection. Staff will be reminded to make sure all diaper cream is labeled with child's name on it.
