Comments:
A monitoring inspection was conducted on 02/21/2024 between the hours of approximately 1:15 pm and 3:00 pm. There were 31 children present, ranging in ages from 5 months to 4 years, with 5 staff supervising. Children were observed during their designated rest period.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There was no medication on site today. A total of 3 child records and 2 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-40-G
Description: Based on children's record review, the center did not ensure to include in the child's record documentation of viewing proof of the child's identity and age within seven business days of initial attendance.
Evidence:
1) The record for Child #3 (start date 07/21/2023) did not contain documentation of viewing proof of the child's identity and age.

Plan of Correction: Birth certificate viewed and recorded.

Standard #: 8VAC20-780-40-M
Description: Based on documentation review, the center did not maintain a current written list of all children's allergies, sensitivities, and dietary restrictions.
Evidence:
1) On the date of inspection, the allergy list available in each room was dated 10/24/2022. The information contained was not current.

Plan of Correction: New allergy list updated and posted. No current allergies in the center.

Standard #: 8VAC20-780-80-A
Description: Based on documentation review, the center did not keep an accurate written record of children in attendance each day.
Evidence:
1) At the time of entrance, the preschoolers and pre-k rooms had been combined for the designated rest period but the attendance record (face-to-name form) had not been updated.

Plan of Correction: Talked with staff of both rooms and went over process to combine rooms on face-to-name.

Standard #: 8VAC20-780-240-E
Description: Based on staff's records review, the center did not ensure that within 30 days of the first day of employment, staff complete orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.
Evidence:
1) On the date of inspection, the record for Staff #1 (start date 01/15/2024) did not contain documentation of having completed orientation training in first aid and CPR.

Plan of Correction: Will have new staff complete CPR/First aid within 30 days.
