Technical Assistance:
Orientation form does not include all required information (updated 10/2021) and can be found on the VDOE Licensing website under CDC/Model forms. Be sure all items are checked off and the form is signed by both the individual receiving the orientation as well as the individual giving the orientation.

Any self carry medication must be inaccessible to other children and staff must be aware of the location of the medication at all times.

Comments:
An unannounced renewal study was completed. Three classrooms were in attendance with Americorp workers and Aspire staff. Reviewed physical space, nutrition (today's meal was hamburger, bean salad and fruit cup) as well as emergency preparation, medication storage and authorizations. Reviewed a sample of children's records, 2 staff records, drills and medication review on-site. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please be aware that the plan of correction must be provided within 5 business days of receipt of the inspection notes if you want it displayed on the website. Standard #: 8VAC20-780-60-A-8
Description: Based on record review, a child with an identified food allergy does not have a written action plan in case of exposure.
Evidence:
No action plan was available for Child #1 who has an identified food allergy with instructions from a physician regarding steps to be taken in case of a reaction.

Plan of Correction: An action plan sheet has been provided to the parent to be completed by the physician.

Standard #: 8VAC20-780-510-G
Description: Based on observation, a medication is not labeled with identifying information.
Evidence:
The inhaler is not in a box with the prescription label containing the student name, dosage amount and times to be given.

Plan of Correction: The parent has been informed.

Standard #: 8VAC20-780-510-L
Description: Based on observation, a medication was not kept in a secure place preventing access by other children. The medication plan, designated by written physician order, was not current.
Evidence:
An emergency medication that a child can self carry was not stored in a separate area away from other backpacks. The physician order to self carry expired 6/2023.

Plan of Correction: A dedicated locked has been assigned for the backpack.

The parent has been reminded to have the physician complete the written order.

Standard #: 8VAC20-780-550-A
Description: The current written emergency plan does not include components that identify staff responsibilities.
Evidence:
The written plan is missing items such as who is responsible for securing essential documents/medications, procedures to reunite with parents and special accommodations for children with special needs during different types of drills.

Plan of Correction: The emergency plan will be updated.
