Comments:
An unannounced monitoring inspection was conducted February 26, 2024 between the hours of 12:00pm - 2:20pm. There were 90 children directly supervised by 13 staff. The physical plant, 5 staff records, 9 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed preparing for nap time, diapering and napping. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 8VAC20-780-130-E
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. Child #2 (dob: 8/29/22) last immunization record on file was dated 3/17/23.
2. Child #4 (dob:7/13/21) last immunization record on file was dated 7/19/22.
3. Child #6 (dob:7/7/20) last immunization record on file was dated 9/14/20.

Plan of Correction: During required file updates that occur every 6 months, management will require parents of children under the age of 2 years old to also turn in updated shot records.

Standard #: 8VAC20-780-160-A
Description: Based on record review the center failed to ensure that tuberculosis (TB) tests or screenings were completed within the last 30 calendar days of the date of employment.

Evidence:
1. Staff #1 had a TB document dated 7/19/23, the listed hire date for this staff was 07/10/23.
2. Staff #2 had a TB document dated 12/19/23, the listed hire date for this staff was 12/15/23.

Plan of Correction: New staff will not be allowed to start working until the TB Test is completed.

Standard #: 8VAC20-780-160-C
Description: Based on record review, the center did not ensure that employees obtain and submit the results of a follow-up tuberculosis (TB) screening at least every two years.

Evidence:
A current TB screening was due for Staff #4 The Tb screening form in the record for Staff #4 was dated 1/16/21.

Plan of Correction: Moving forward, if current staff do not submit their negative TB test results by the time they are expired, staff will not be allowed to work until they have submitted a negative test. Staff# 4 submitted a negative TB test on 2/29/24.

Standard #: 8VAC20-780-70
Description: Based on record review, the center failed to obtain documentation to demonstrate that the individual possesses the education or certification required by the job position.

Evidence:
There was no documentation in the staff record that Staff #2 possessed the education or certification required by the job position.

Plan of Correction: New staff will be required to provide documentation of education on their first day of employment. If not submitted by end of first day of employment, staff will not be allowed to return until documentation is submitted.

Standard #: 8VAC20-780-240-B
Description: Based on record review, not all records contained documentation that orientation training had been completed within the first seven days of assuming job responsibilities.

Evidence: The record for staff #3 contained documentation of orientation training dated 10/19/23 the listed hire date for this staff was 10/2/23.

Plan of Correction: All management will be trained on delivering orientation training to ensure it is conducted within the first seven days of employment. Training will happen by 3/8/24.

Standard #: 8VAC20-780-240-E
Description: Based on record review, the center failed to ensure staff completed orientation training in first aid and cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care within 30 days of the first day of employment.

Evidence:
Staff #2 had no documentation of orientation training in first aid and CPR.

Plan of Correction: Moving forward, management will ensure First AID and CPR training is completed within the first 30 days of employment. CPR and First class has been scheduled for 3/6/24.
