Comments:
An unannounced monitoring inspection was conducted on 02/26/2024 from 12:30pm to 3:20pm. There were 159 children ages 6 weeks to age 5 years old supervised by 20 staff. The physical plant, required postings, programming, attendance, menus, 3 staff records, 2 children?s records were reviewed. Children were observed participating in nap, tummy time, floor play, quiet activities, and tablet time. Diapering and handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on review of 3 staff records, the provider did not obtain documentation of the results of a national fingerprint background check prior to date of hire.

Evidence: The record for Staff #1 (Date of Hire 2/19/24), did not contain documentation of a fingerprint background check completed prior to their date of hire. The Fingerprints in the file were completed on 2/20/24.

Plan of Correction: Management will not allow returning staff to participate in all teacher training until all background checks have been renewed.

Standard #: 8VAC20-770-60-B
Description: Based on review of 3 staff records, the center did not obtain documentation of a completed sworn disclosure statement for each staff prior to date of hire.

Evidence: The record for Staff #1 (date of hire 02/19/24) did not contain documentation of a completed sworn disclosure statement prior to the date of hire. The Sworn disclosure statement in the file was completed on 2/20/24.

Plan of Correction: Management will not allow returning staff to participate in any all teacher trainings prior to them signing paperwork.

Description: (Repeat Violation) Based on observation, the center did not ensure that hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
During inspection in the kitchen area directly off the main hall where the door was open, a jug of disinfectant was stored on the floor.

Plan of Correction: The kitchen will be locked to prevent access by children.

Description: (Repeat Violation) Based on observation, the Center did not ensure that cleaning and sanitizing materials were not located above food, food equipment, utensils or single-service articles and should be stored in areas physically separate from food.

During inspection in the Preschool 3A classrooms, there were cleaning and sanitizing materials stored together with a box of crackers.

Plan of Correction: All cleaning material will be stored away from all food items.

Standard #: 8VAC20-780-340-F
Description: Based on observation, the center did not ensure that children under 10 years of age always be within actual sight and sound supervision of staff.

Evidence:
During inspection in the Nursery 2 classroom (ages 11 months-18 months), there were 4 teachers sitting on the floor with 13 Infants and Toddlers who were resting on 1 side of a half wall that divides the room. On the other side of the half wall there was a sleeping infant (age 11 months) in a crib and there were no staff present in this area. The child was unable to be seen by any staff member in the classroom.

Plan of Correction: All children will always be within sight and sound supervision of staff.

Standard #: 8VAC20-780-440-J
Description: Based on observation, the center did not ensure that there was thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied cribs.
Evidence: In the Nursery 2 classroom, there was a sleeping infant in a crib where the service side and walkway was only 24 inches from another crib.

Plan of Correction: 30" of space will be available on service side of all cribs that are occupied.

Standard #: 8VAC20-780-450-A
Description: Based on observation, the center did not ensure that cribs, cots, mats and beds used by children other than infants during designated rest periods should have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
Evidence: In the Preschool 3A classroom, there were 2 children sleeping on mats without a top cover or without a one-piece covering open on 3 edges.

Plan of Correction: Center will provide top and bottom cover if families forget to provide them on any day.

Standard #: 8VAC20-780-500-B
Description: (Repeat Violation) Based on observation, the center did not ensure that the diapering surface should only be used for diapering or cleaning children.
Evidence: In the Preschool 2B classroom on the diapering surface was stored a bag of clothing and a roll of tape.

Plan of Correction: Diapering surfaces will not be used for storage.
