Technical Assistance:
Consultation was given on additional staff having MAT training as a child with a prescription medication must be in the care of a MAT trained staff. The center should have at least 2-3 trained staff in case staff are absent or on leave.

CPR certification must contain an in-person component to demonstrate knowledge.

Comments:
An unannounced monitoring visit was conducted. Five classrooms were present with children in direct care of staff and in ratio for supervision. Observed infant care, diapering, labeling of bottles with children's names and date, preparation for lunch time, hand washing, circle time and lunch. Lunch served today was fish sticks, fries and carrots with milk. One emergency medication was on-site with one staff trained in Medication Administration training (MAT) certification. Emergency preparation is current for drills and staff trained in first aid and CPR. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-60-A-8
Description: Based on record review, an allergy care plan was not available for a child with a diagnosed food allergy.
Evidence:
No written care plan was available for Child #1 to include instructions from a physician regarding the food to which the child is allergic to and the steps to take in case of a suspected or confirmed allergic reaction.

Plan of Correction: The Child's parents bought in the renewed FARE form.

Standard #: 8VAC20-780-80-A
Description: Based on observation and record review, the center did not maintain daily attendance that documents the arrival of each child as it occurs.
Evidence:
The Infant attendance record did not account for four children present in the classroom as they were not listed on the attendance as of 12:00 pm.

Plan of Correction: The school uses an electronic attendance system and all children clocked in. Teachers are re-trained to maintain a hard copy version.

Standard #: 8VAC20-780-240-A
Description: Based on record review and staff statement, an employee has not completed the Virginia Department of Education sponsored orientation course within 90 days of employment.
Evidence:
Staff #1 was hired 11/9/2023 and does not have a certificate of completion in her personnel record. An administrative staff confirmed that the staff has not completed the training within the 90 day time frame.

Plan of Correction: The staff member has completed the training.
