Comments:
An unannounced monitoring inspection was conducted on February 26, 2024, from 11:40 a.m. through 1:20 p.m. Upon arrival, there were 79 children, ages 4 months through 5 in the care of 17 staff.

The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies. A total of 5 children and staff records were reviewed.

Information gathered during the inspection determined noncompliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

Standard #: 8VAC20-780-150-B
Description: Based on record review, one immunization record did not include a signature and date by a physician, his designee or an official of a local health department.

Evidence:

1. Five children records were reviewed.
2. Child # 5 did not have documentation of immunizations being signed and dated by a physician, his designee or an official of a local health department.
3. Staff confirmed the immunization record received did not have documentation of being signed and dated by a physician or designee.

Description: Based on record review, the center failed to obtain proof of identity for one child.

Evidence:

1. Five children records were reviewed.
2. Child # 5 did not have documentation of reviewing proof of identity.
3. Staff confirmed proof of identity had not been reviewed for Child #5.

Description: Based on observation the center failed to have a bottom cover on two cots.

Evidence:

1. Observation of the toddler classroom revealed two cots did not have a bottom sheet while children were napping.
2. Staff confirmed there were not bottom sheets for the two cots.

Description: Based on review of medication the center failed to have one emergency medication with the pharmacy label attached.

1. Auvi -Q for Child #6 did not contain a pharmacy label to include the child's name, the name of the medication, dosage amount, and times or times to be given.
2. Staff confirmed Auvi-Q was not received from the parent with the pharmacy label attached.

Description: Based on review of medication, the center failed to return medication to the parent or dispose with 14 days of authorization expiring.

Evidence:

1. The medication and medication paperwork was reviewed for Child #7.
2. Parent authorization to administer medication expired on 01.22.2024.
3. Staff confirmed the medication had not been returned to the parent or disposed of within 14 days of the authorization expiration date.
