Comments:
An unannounced on-site monitoring inspection was conducted on 02/07/2024. There were 33 children, ages 10 months-5 years, and 5 staff members present during the inspection. The inspector observed the following: free choice activities, preparing for lunch and nap. A total of 5 children?s records and 5 staff records, were reviewed. The inspectors discussed the following with staff: attendance requirements, program leader. The inspector arrived at 10 AM and departed at 12:45 PM.

Standard #: 8VAC20-780-60-A
Description: Based on record review, the center failed to ensure that the separate record for each enrolled child shall contain all the elements as required by the standards.

Evidence:

1. The record for Child #3 did not include the following information Allergies and intolerance to medication or any other substances, and actions to take in an emergency situation, and Chronic physical problems and pertinent developmental information and any special accommodations needed.

Description: Based on review of staff record, the center failed to ensure that all records were complete per the standard.

Evidence:

1.Staff # 2 had a hire date of 03/18/2018 and was missing the following items: job title and did not have documented of any health problems that may interfere with fulfilling the job responsibilities.
2.Staff # 3, 4 and 5 were missing the following: did not have documented of any health problems that may interfere with fulfilling the job responsibilities.

Description: Based on staff record review, and interviews, the center failed to ensure that each grouping of children had a qualified program lead.

Evidence:

1.Staff # 2 record was reviewed and there were no documentation of educational requirements and a high school diploma or equivalent for a program leader position.
2.Staff # 3 and #4 records were reviewed and there was no documentation of a high school diploma or equivalent for a program leader position.
3.Staff #5 record was reviewed and there was no documentation of experience, educational requirements and a high school diploma or equivalent for a program leader position.

Description: Based on observation, the center failed to ensure that all chemicals were locked using a
safe locking method that prevents access by children.

Evidence:

1. In the infant room sitting on the counter was a Febreze aerosol can.
2.In the school age area and where the preschool children sleep was a container of hard surface sanitizer sitting under the desk that was unlocked.

Description: Based on observation, the center failed to ensure that all food brought from was clearly dated ad labeled in a way that identifies the owner.

Evidence:

There were seven lunches in the 2 year old room that were not clearly dated and labeled in a way that identifies the owner.

Standard #: 8VAC20-780-560-J
Description: Based on observation, the center failed to ensure that tables and highchairs trays were sanitized before and after each use for feeding.

Evidence:

The highchair trays in the toddler room were not sanitized prior to eating lunch. The highchairs were brought out of the closet and toddlers were placed in the highchair without sanitizing.
