Technical Assistance:
The following standards were discussed with the center director during the inspection: 8 VAC 20-780-60 (children's records), 8 VAC 20-780-530 (first aid training and CPR), 8 VAC 20-780-550-P (children's injury reports).

Comments:
An unannounced renewal inspection was initiated and concluded on 2/28/24 from 1:00pm until 3:15pm. At the time of the inspection, there were 10 children in care with 5 staff present. Two additional children arrived at the center from school during the course of the inspection. A sample of 5 children's records and 4 staff records were reviewed. Children were observed resting quietly during nap time, eating snack and engaging in free play. First aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills and required center postings were reviewed. There are currently no children in care who require medication administration. Information gathered during the inspection determined non-compliance with applicable standards or law. Violations were documented on the violation notice issued to the program and discussed with the center director during the exit interview.

Violations:
Standard #: 8VAC20-780-240-A
Description: Based on record review and interview, the center did not ensure that staff shall complete the Virginia Department of Education-sponsored orientation course within 90 calendar days of employment.

Evidence:
1. The records for staff #1 (date of employment: 9/28/23) and staff 2 (date of employment: 9/18/23) do not contain documentation that they have completed the Virginia Department of Education-sponsored orientation course.
2. The center director confirmed that staff 1 and staff 2 have not completed the DOE-sponsored orientation course within 90 days after employment.

Plan of Correction: The center responded with the following: Staff 1 and staff 2 will complete the VDOE-sponsored orientation training by 3/4/24. If not completed by that date, they will not be able to return to work until the training is completed.

Standard #: 8VAC20-780-440-E
Description: Based on observation, the center did not ensure that there shall be at least 12 inches of space between occupied cots.

Evidence:
In the infant room, there were 2 occupied cots with only 5 1/2 inches of space between them, where 12 inches are required.

Plan of Correction: The center responded with the following: Staff will be reminded about appropriate spacing of cots and the room will be rearranged to allow for better spacing of cots during nap time.

Standard #: 8VAC20-780-450-A
Description: Based on observation and interview, the center did not ensure that cots used by children during the designated rest period shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.

Evidence:
1. During the designated rest period, the licensing inspectors observed two children on cots with no bottom cover, one child on a cot with no top cover and one child on a cot with no top or bottom cover.
2. Center staff confirmed that not all cots occupied by children during the designated rest period had top and bottom covers.

Plan of Correction: The center responded with the following: Parents will be reminded to bring cot linens back after being sent home for cleaning. Center management will purchase extra linens to be used in case a child does not have them.

Standard #: 8VAC20-780-570-E
Description: Based on observation, the center did not ensure that prepared infant formula shall be dated.

Evidence:
In the infant classroom, there were 3 bottles prepared with formula that were not dated.

Plan of Correction: The center responded with the following: Staff do their best to ensure that bottles are labeled with the child's name and date; however, it often is rubbed off during the course of the day or parents forget to date them. Staff will be reminded to check bottles daily to ensure they are not only labeled with the child's name but dated. If the parent had not dated the bottle, masking tape will be used to write the date on the bottle.
