Comments:
An unannounced renewal inspection was conducted 2/7/2024 from 10:55am through 12:10pm. There were 22 children directly supervised by 4 staff and 1 volunteer and both classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 1 staff record, 1 volunteer record, 5 children?s records, 3 medications, emergency drills, injury reports, emergency supplies, and policies were inspected. Children were observed eating lunch, completing puzzles, doing sorting and tracing activities, learning about the continents, cutting and shaping play dough, and painting with water colors. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. There was an adequate number of CPR/First Aid, Daily Health Observation, and MAT trained staff on site at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Description: REPEAT VIOLATION

Based on record review, the center did not obtain the results of a Virginia Child Abuse and Neglect Central Registry search within 30 days of volunteer service for all volunteers

Evidence: Volunteer #1 (Start Date: 9/7/2023) did not have Central Registry results on file.

Plan of Correction: Send for central registry.

Description: Based on record review, the center did not ensure that children's records contained all required information.

Evidence: Child #1's record did not have documentation of the center viewing proof of the child's identity and age.

Description: REPEAT VIOLATION

Based on record review, the center did not obtain a written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence: Child #6 has a diagnosed food allergy and does not have a written allergy care plan on file.

Description: Based on review of medications and documentation, the center did not obtain parental authorization to administer medications for all medications maintained on site.

Evidence: Child #6 has an emergency medication on site for a food allergy and there is no parental authorization on file granting the center permission to administer the medication.

Plan of Correction: Get with parents to give dr note.

Description: Based on record review and staff interview, the center did not ensure that shelter in place procedures were practiced a minimum of twice per year.

Evidence: There has been only one shelter in place drill conducted within the last year.
