Technical Assistance:
Discussed medication authorization forms. Authorization forms should match the medication dose of the medication present on site. Medication authorizations should also contain a beginning and end date to determine the duration of the authorization.

Comments:
An unannounced renewal inspection was conducted today from 7:30am-8:30am. At the beginning of the inspection there were 16 children directly supervised by 3 staff. The physical plant, 3 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed arriving for the day, playing board games, eating breakfast, and dismissing to school. Hand-washing procedures were also observed. There was an assortment of age appropriate materials for the children in care. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Description: Based on record review, children's records did not contain all required information.
Evidence:
Child 9's record did not contain the phone number of a second emergency contact.

Plan of Correction: Followed up with parent. Number has been updated in file.

Standard #: 8VAC20-780-510-P
Description: Based on a review of medication, medication was kept onsite for more than 14 days after the medication authorization had expired.
Evidence:
Child 5's medication was present on the date of inspection. The medication authorization for Child 5's medication expired at the end of the 2022-2023 school year.

Plan of Correction: Expired medications returned to parent. Will follow up with parent to bring in new unexpired med if needed.
