An unannounced monitoring inspection was conducted on 2/16/2024 from 3:56 to 6:38 pm. There were 32 children in care at the start of the inspection, ages six years to twelve years, in five groups under the care of five staff. The Licensing Inspector reviewed two children?s records, four staff records, and other program records. The children were observed doing quiet activities, working on computers, and playing in the gym.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-160-A Description: Based on record review, the center failed to obtain TB screenings for new staff no older than 30 days prior to employment.

1. Staff 2 has a documented employment date of 10/13/2023 and the TB screening was dated 7/22/2022.

Based on interview and staff file review, the center failed to have a qualified director.
EVIDENCE:
