Comments:
An unannounced monitoring inspection was conducted on 2/27/2024 from 4:30pm to 5:30pm. At the time of the inspection there were 20 children in care and 3 staff members. Children were observed participating in various activities including group choice games and homework. Records were reviewed for 5 children. Staff records were reviewed at the main office location on 2/28/2024. Evacuation drills, emergency supplies and other required records and postings were also reviewed. There was no medication on site during this inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit interview.

Violations:
Standard #: 8VAC20-780-60-A
Description: Repeat Violation
Based on record review and interviews, it was determined the center did not maintain and keep at the center a separate record for each child enrolled with all required information.
Evidence:
1.There was no record available at the center for child #2.
a.This child was present and in care during the inspection.
b.Staff #1 confirmed there was no record for child #2.

2.The record for child #5 did not contain the date of enrollment.
a.Staff #1 confirmed the date enrollment was not documented.

Standard #: 8VAC20-780-70
Description: Based on observation and interview, it was determined that the center did not ensure that the name, address, and telephone number of a person to be notified in an emergency was kept at the center for each staff member.
Evidence:
1.During the inspection, staff were unable to provided documentation of emergency contacts for staff members.

Plan of Correction: Staff located the emergency contact information following the inspection and provided documentation to the inspector.

Standard #: 8VAC20-780-540-E
Description: Repeat Violation
Based on observation and interviews, it was determined the facility did not ensure the required nonmedical emergency supplies were available on each floor of the building.
Evidence:
1. The battery-operated radio was missing batteries and did not work.
2.Staff #1 confirmed the radio was not working.
