Comments:
A monitoring inspection was conducted on February 20, 2024 with the center director and staff. There were 60 children present, ranging in ages from 8 months to 12 years, with 8 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed. The children were having rest time while the school agers had a speaker for career day.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on review of six children?s records, the facility failed to have a physical examination for each child within 30 days of attendance. Evidence: Child 5 (date of enrollment 9/18/2023) did not have documentation of a physical on file.

Plan of Correction: Staff will request the physical from the parent.

Standard #: 8VAC20-780-160-A
Description: Repeat violation. Based on review of 6 staff records, the center failed to obtain documentation of a negative tuberculosis screening or test (TB) in the required timeframe. The test or screening must be done at the time of employment, prior to coming in contact with children, and no more than 30 days prior to of the date of employment. Evidence: Staff 1 (date of employment 4/24/2022) did not have a TB test until 6/10/2022, Staff 2 (date of employment 1/20/2022) did not have a TB test until 4/2/2022, Staff 3 (date of employment 7/10/2023) had a TB test 9/19/2022, and Staff 4 (date of employment 6/06/2023) did not have a TB test until 7/19/2023. Staff 6 (date of employment 1/29/2024) did not have documentation of a TB test or screening on file.

Plan of Correction: TB tests will be conducted no more than 30 days prior to employment and prior to direct contact with children.

Standard #: 8VAC20-780-260-A
Description: Based on review of inspection reports, the facility failed to ensure that an annual fire inspection was conducted by the appropriate fire official having jurisdiction. Evidence: There was not a current fire inspection on file.

Plan of Correction: Management will contact the fire marshal for the latest report.

Standard #: 8VAC20-780-280-B
Description: Repeat violation. Based on observation, the facility failed to keep hazardous substances in a locked place that prevents access by children. Evidence: There was unlocked sanitizer and Lysol in cabinets above the diapering area in the infant room, toddler room, and twos room.

Plan of Correction: Locks will be put on the cabinets containing hazardous substances.

Standard #: 8VAC20-780-350-B-5
Description: Based on observation, the facility failed to maintain the required ratio of 1 staff for every 18 children age 5 years up to 9 years. Evidence: On 2/20/2024, there was one staff alone with 24 school age children upon licensing inspector for at least 15 minutes.

Plan of Correction: Corrected during inspection. Management stepped in the room to maintain ratio.

Standard #: 8VAC20-780-570-B
Description: Based on observation and interview, it was determined that the center did not ensure that bottles not be used while the child is in his designated sleeping location.
Evidence: In the infant room, two children under the age of 12 months were observed to have a bottle while lying down inside their (crib) designated sleeping area.

Plan of Correction: Corrected during inspection. The bottles were removed from the cribs and staff were instructed to keep bottles out of designated sleeping areas.
