An unannounced monitoring inspection was conducted today from 11:07am to 11:45am. At the time of entrance, 41 children were in care with 3 staff members present. The physical space, emergency supplies, evacuation drills, the allergy action plans, medication, the Emergency Preparedness Plan, 4 children's records and 3 Staff certification for CPR/First Aid and Mat training were reviewed. Ratios were in compliance and required documents were posted. The site was clean and organized. Interactions between the children and staff were positive. The children were playing with games, reading, working on art projects, washing hands and eating lunch. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names, list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Standard #: 8VAC20-780-130-A Description: Repeat Violation
Based on review, the center did not obtain documentation that each child had received the
immunizations required by the State Board of Health before the child attends the center.
Evidence:
1. Child #2 (start date 9/18/2023) did not have the child's immunization record in the file and
the school office was closed during the inspection.Plan of Correction: Reached out to parents requesting epi pen asap.
Standard #: 8VAC20-780-140-A Description: Based on review of four children's records, the center did not obtain a copy of one child's physical prior to enrollment or within 30 days.
Evidence:
1. Child #2 (start date 9/08/2023) did not have a copy of the child's physical and the school office was not open.Plan of Correction: -email office for copy of health form

Based on record review, one of four children's records did not have complete information.

1. Child #2's (start date 9/18/2023) file was missing one emergency contact address, immunization record and health exam..Plan of Correction: -emailed office to request a copy of health form

Based on review, the center did not obtain a written care plan for each child with a
diagnosed food allergy, to include instructions from a physician regarding the food to which
the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic
reaction.
Evidence:
1. Child #2 (start date 9/18/2023) had a diagnosed food allergy and the center did not
obtain a written care plan for the child to include instructions from a physician regarding

or confirmed allergic reaction.Plan of Correction: contact parents and explain why we need the epi pen and why sacc got a violation cause of it. Speaking to the parents they both made Dr. Appt. This shoutd be resolved by April 5th
