Comments:
An unannounced monitoring inspection was conducted on 2/23/2024 from 1:40 pm to 5:04 pm. There were 32 children in care at the start of the inspection, ages three months to four years, in five groups under the care of eight staff. Four child records, four staff records, background checks on four new staff members, three medications and associated records, and other documents were reviewed. Children were observed napping, snacking, and in free play.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 22.1-289.035-B-2
Description: Based on record review, the center failed to obtain a Sworn Disclosure statement for a staff member prior to their first date of employment.

EVIDENCE:
Staff 6 has a documented employment date of 2/6/2024 and the sworn disclosure statement was signed on 2/23/2024.

Plan of Correction: Director will ensure Sworn Disclosure is signed on r before all new hire's first day of employment.

Standard #: 8VAC20-780-80-A
Description: Based on observation and record review, the center failed to maintain a written record of daily attendance that documented the arrival and departure of each child in care as it occurred.

EVIDENCE:
1. The infant room had seven children in care and the attendance sheet showed there were eight children.

2. The room with children ages 12 months to 16 months had seven children, and the attendance sheet showed there were six children.

3. The room with children ages 16 months to 24 months had nine children and the attendance sheet showed there were 11 children.

Plan of Correction: Center will provide training to ensure proper attendance documentation procedures are being followed as children arrive and depart.

Standard #: 8VAC20-780-340-D
Description: Based on observation and record review, the center failed to have a staff member who meets the qualifications of a program leader or program director in each group of children.

EVIDENCE:
While the children were awake, they were in the care of Staff 1, 2, 3 at various times. Review of staff records show that none of these staff people met the qualifications of a program leader or program director.

Plan of Correction: Director will train all teachers and new hires to have the qualifications of a program leader.

Standard #: 8VAC20-780-510-P
Description: Based on record review and medication review, the center failed to dispose of medication or return it to the parents within 14 days of the permission to administer it expired.

EVIDENCE:
1. Permission to administer Medication 2 expired on 1/17/2024 and the medication is still in the center.

2. Permission to administer Medication 1 expired on 2/22/2024 and the medication is still in the center.

Plan of Correction: Center will ensure that medications will be disposed of or returned to parent within 14 days of the permission to administer expiring.

Standard #: 8VAC20-780-560-B
Description: Based on observation, the center failed to have a non-absorbent surface to use to change diapers.

EVIDENCE:
The changing pad in the room with 16 months to 24 months aged children was ripped, and therefore non-absorbent.

Plan of Correction: Center will ensure changing mat is replaces or has a non-absorbent covering for diapering procedures.

Standard #: 8VAC20-780-560-G
Description: Based on observation, the center failed to label food from a child?s home with the name and date.

EVIDENCE:
Food from home was not labeled with a name or date. Staff identified the food as belonging to Child 2.

Plan of Correction: Staff will be retrained on proper food labeling procedures.
