Technical Assistance:
Consultation was given on MAT and FA/CPR training dates/trainers and model forms on the VA DOE Licensing Website.

Infants sheets need to be tight fitting and no pacifiers that are attached to small stuffed animals should be used while a child is sleeping.

Comments:
An unannounced monitoring inspection was conducted. Five classes were present with direct care staff supervising children. Observed children transitioning out to the playground and back into the center, infant care, circle time and singing/letter review. Reviewed physical space, records for two newly hired staff, four children's records and medications on-site, authorizations and storage of emergency medication, drills, trainings and nutrition. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard #: 8VAC20-780-280-B
Description: Based on observation, cleaners were not kept in a locked place using a safe locking method to prevent access by children.
Evidence:
A spray bottle of cleaners was on an open shelf in the Two's classroom.

Plan of Correction: Shelf was adjusted in the Two's classroom and the spray bottle was placed in the locked cabinet.

Standard #: 8VAC20-780-280-G
Description: Based on observation and staff administration statement, the hazardous substance was not kept in the original container and the substitute container was not clearly labeled with the contents.
Evidence:
The spray Windex bottle was not labeled with the additional cleaner it contained. (Pine-Sol)

Plan of Correction: Staff administration labeled the container in the Two's room during the inspection.

Standard #: 8VAC20-780-510-E
Description: Based on medication review, an emergency medication authorization has expired from both the parent and physician.
Evidence:
Child #1's authorization form for an emergency medication expired 1/23/2024 from the physician and 1/1/2024 from the parent.

Plan of Correction: Sent to parent to complete.
