An unannounced complaint inspection was conducted on 2/16/2024 from 12:00pm to 12:30pm, and 2/28/2024 from 12:30pm to 2:35pm in response to a complaint that was received by the department regarding Administration, Staff Qualifications and Training, Staff and Supervision, Programs, Special Care Provisions and Emergencies, and Special Services. Staff interviews, and review of documents were made. The preponderance of evidence does support a valid finding regarding standards related to Administration, Staff Qualifications and Training, Staffing and Supervision, Programs, Special Care Provisions and Emergencies, and Special Services. Based on review of staff files, documentation of a negative tuberculosis screening was not completed within 30 calendar days of the date of employment.
Evidence: Staff #2 did not have documentation of a tuberculosis screening within30 calendar days of the date of employment. Based on interviews with the director and staff members, there was not always one staff member who meets the qualifications of a program leader and an immediately available staff member during the stated hours of operation when one or more children were present.
