Comments:
An unannounced complaint inspection was conducted on 2/16/2024 from 12:00pm to 12:30pm, and 2/28/2024 from 12:30pm to 2:35pm in response to a complaint that was received by the department regarding Administration, Staff Qualifications and Training, Staff and Supervision, Programs, Special Care Provisions and Emergencies, and Special Services. Staff interviews, and review of documents were made. The preponderance of evidence does support a valid finding regarding standards related to Administration, Staff Qualifications and Training, Staffing and Supervision, Programs, Special Care Provisions and Emergencies, and Special Services. Based on review of staff files, documentation of a negative tuberculosis screening was not completed within 30 calendar days of the date of employment.

Evidence: Staff #2 did not have documentation of a tuberculosis screening within30 calendar days of the date of employment. Staff #3 began employment 9/5/2023 and the documentation of a negative tuberculosis screening was dated 3/11/2023.

Plan of Correction: Center will ensure that all staff submit documentation of tuberculosis screening within 30 calendar days of the date of employment.

Based on interviews with the director and staff members, there was not always one staff member who meets the qualifications of a program leader and an immediately available staff member during the stated hours of operation when one or more children were present.

Evidence: It was determined that on 2/15/2024, 2/16/2024, and 2/27/2024 there was only one staff member present at the center at opening while children were present due to a staff member arriving late.

Plan of Correction: The center will ensure there is always at least 2 staff members on the premises during operation.

Plan of Correction: The center will ensure that infants are provided with daily outdoor time when weather and air quality allow.

Evidence: Staff #2 and Staff #4 had a current certification in CPR that was missing an in-person competency demonstration.

Based on interview with center director, the menu listing foods to be served for meals and snacks did not list substitutes food.

Evidence: The center director confirmed that although parents were notified about food substitutions digitally, the posted menu was not updated.

Plan of Correction: The center will update the menu with any substituted food as this occurs.
