Technical Assistance:

1. Discussed requirement for most recent inspection findings to be posted in the program area (violation notice).

Comments:

Conducted an unannounced renewal inspection at 1:55pm. Observed 16children + 3 direct-care staff. Ratios and supervision were in compliance. Children were observed arriving make clay projects, watching a movie and playing on electronic devices. The program areas were found to be clean and sufficiently supplied with toys and equipment for the children. Areas of non-compliance with standards and laws reviewed were found. Violations:

Standard #:	8VAC20-780-130-A
Description:	Repeat violation and systemic deficiency. Based on records reviewed, 1 of 4 children's records did not include documentation of immunizations prior to the child attending. Evidence: The record for Child #4 did not include a immunizations.
Plan of Correction:	Staff contacted the family about this paperwork initially and the mother who has language challenges sent the wrong forms. We're working with the families to get the correct form from the child's base school.
Standard #:	8VAC20-780-140-A
Description:	Based on records reviewed, 1 of 4 children's records did not include documentation of a physical exam within the first 30-days of enrollment. Evidence: The record for Child #1 did not include a physical exam.
Plan of Correction:	The child had all health and immunization forms except for the cover sheet to the physical. Staff has contacted the family and have requested this form from them.
Standard #:	8VAC20-780-280-B
Description:	Based on observations made, hazardous substances were not kept in a locked place. Evidence: An assortment of cleaning supplies were observed on an open shelf within reach of the children in a storage closet. The door to the storage closet was propped open, the cleaning supplies were approximately 2' inside the door and children were observed going into the storage closet.
Plan of Correction:	Staff will place cleaning supplies on the designated shelf and will no longer prop the closet door open. This is not a regular practice at our center.
Standard #:	8VAC20-780-550-P
Description:	Based on records reviewed, 3 of 7 written injury reports did not include required information. Evidence: 3 reports did not document the time the parent was notified of the child's injury.
Plan of Correction:	Staff will review and make sure to enter time parent was notified. 2 of these were during our full day program in which the staff weren't staff that typically work at our center.
