A monitoring inspection was initiated on 3/5/2024 and concluded on 3/5/2024. There were 38 children present with eight staff supervising and the program director was present. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and two staff records were reviewed. The inspection started at9:00 am and concluded at 11:45 am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 8VAC20-780-140-A Description: (REPEAT VIOLATION) Based on record reviews the center failed to ensure that each child shall have a physical examination by or under the direction of a physician; before the child's attendance or within 30 days after the first day of attendance.
Evidence:
Child #1 does not have a physical examination on file.Plan of Correction: The director contacted the parent and had the physical sent over to the center.
