Technical Assistance:
Discussed classroom attendance record form change to record in and out, for direct care providers to have in the classroom.

Comments:
An unannounced monitoring inspection was conducted today from 12:30pm-1:25pm. There were 24 children directly supervised by 3 staff. The physical plant, 2 staff records, 4 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed quiet reading, napping, laying quietly during rest time. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice.

Based on file review and interview, sworn statement or affirmation forms were not updated every five years.
Evidence:

Description: Based on record review and interview, emergency evacuation, shelter in place, and lockdown drills did not contain all required information.
Evidence:

and lockdown drills did not note the time it took to complete the drill.
2. Emergency evacuation drills, did not note weather conditions.

Plan of Correction: Drill form to be updated and staff will record all required information on the drill form.
