Technical Assistance:
Provided technical assistance on standard:
1) 500B: The diapering area shall have the following: a non-absorbent surface for diapering. For children younger than three years, this surface shall be a changing table or countertop designated for changing (not standing up in the bathroom)
2) 560F: the facility shall post a current menu listing the foods to be served for snack
Discussed policies and procedures, updated standards (2021) and model forms (DOE, not DSS.)

Comments:
An announced initial inspection was conducted from 9:45 am - 12:45 pm with management staff and the center owner in response to a planned relocation of the licensed program. No children were present during this inspection. A qualified director is in place and was present during the inspection. All six classrooms were observed and measured for capacity. The center is well-equipped with a variety of age and stage appropriate furnishings, equipment, and materials. The four cribs meet Consumer Product Safety Commission (CPSC) federal safety standards for cribs (manufactured 9/2023). One staff record and two applicant/agent records were reviewed. The center's policies and procedures and emergency supplies were inspected and the outdoor area was observed.

Standard #: 8VAC20-780-40-K
Description: Based on review of documentation, the facility did not have written procedures for the prevention of shaken baby syndrome or abusive head trauma, including coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness. Evidence: the facility did not have written procedures that included coping with crying babies, safe sleeping practices, and sudden infant death syndrome awareness.

Plan of Correction: Written procedures have been updated and now include all required components.

Standard #: 8VAC20-780-270-A
Description: Based on observation, it was determined that not all areas inside the center were maintained in a safe and operable condition. Evidence: In the infant classroom, there were four cribs with loose fitting sheets. Loose crib sheets are a suffocation hazard and increase the risk of sudden unexplained infant death.

Plan of Correction: Tight fitting sheets have been ordered and will be in place when infants are placed in cribs.

Standard #: 8VAC20-780-330-A
Description: Based on observation, and interviews, the playground is not located and designed to protect children from hazards. Evidence: The playground location is unfenced in the parking lot, with cones as a border, and is in close proximity to a busy three lane road, with a speed limit of approximately 35mph.

Plan of Correction: Children will participate in alternative and teacher-lead activities outside to include: sidewalk chalk, nature walks, bubbles, games, arts and crafts, etc.

Description: Based on observation, the diapering area did not contain all required items. Evidence: there was not any soap or disposable towels by the sink in the infant classroom.

Plan of Correction: Soap and disposable towels were placed by the sink during the inspection.

Standard #: 8VAC20-780-540-C
Description: Based on observation, the facility did not have all the required items in the first aid kit. Evidence: there were no triangular bandages in the first aid kit.

Plan of Correction: Triangular bandages have been ordered.

Standard #: 8VAC20-780-550-B
Description: Based on review of documentation, the facility did not have an emergency preparedness plan that contained all required procedural components. Evidence: the emergency preparedness plan did not have procedural components for: a) methods to ensure any health care needs to include medications and care plans, contact information for staff, and supplies are taken to the assembly point or relocation site; b) accommodations or special requirements for infants, toddlers, and children with special needs to ensure their safety during evacuation or relocation.

Plan of Correction: The emergency preparedness plan has been updated and now includes all required procedural components.

Description: Based on observation, and interview, the facility did not have a one-day's emergency supply of disposable bottles, nipples and commercial formulas.

Plan of Correction: Emergency feeding supplies for infants will be purchased.
